How do you format a .csv file to import selected e-mail addresses?

What format should a .csv or .vcf file be in to import correctly. Are there field tags to show where the e-mail address should be positioned, for instance, in the comma-delimited data list? should a ,NULL, or ,, be used where there is no data for a field like the First Name?

I recommend following our full walk-through guide on how to "Import Email Accounts/Forwarders in cPanel" It explains the delimiters, and how to access the example .csv or .xls file from cPanel.
