Insecure <form> call error in index.html file

When I run my domain name via whynopadlock I get the error listed above and I see no padlock on my site.

Hello, Thank you for contacting us about an "Insecure call error in index.html file." When I looked at your certificate, it seems to be signed with outdated protocols. Specifically, you are using sha-1, it should be sha-2 instead. I recommend re-keying your CSR with this updated cipher suite, and regenerating/reinstalling your SSL.
