Contact Form Not Sendting

Hi, I put up an email form - www.mediqwest.com/mail.html - and it looks good, validates and returns the thank you message. The issue is it does not send the information. If someone can look at the html file and its associated file ajaxSubmit.php and advise me where I went wrong I would be forever grateful.
