Address Form on PrestaShop Not Showing Form

I am having an issue with my PrestaShop store. When a customer goes to add a new address it takes them to the page where the form usually resides but the form is not shown. Any ideas as to why this might be?

Hello Vault,

Sorry for the problem with the Address form not showing. It sounds like the form is not where it's supposed to be. Either, the form is gone or the link being used to bring you to the form is wrong. You will need to double-check them within your PrestaShop installation. If you're doing trying to do something beyond the normal functionality of the site, then you will need to consult with a developer or the PrestaShop Support community forum.
