How to make blank boxes on Mediawiki ContactPage extension ?

If I follow the instruction, it shows my account name in the box for "your name" and my email address in the box for "Your email address". I do not want to have any of my information here. Instead, I want to make the boxes blank since this information is for sender. I appreciate your help!
