I followed your code exactly at http://www.inmotionhosting.com/support/edu/website-design/using-php-and-mysql/how-to-create-a-custom-php-contact-form and the only change I made was to change my email address to something meaningful. Result? Not sure...but it did say that it sent email...but of course I never got it. Been wondering why ???????

Thank you for contacting us about coding a custom Form Mail. First, make sure you are checking your spam, or Trash box in case a filter is catching the email.

Next, confirm there aren't any typos, or misspellings in your Email Settings.

Then, review the email logs for any evidence of transmission (Live Support can help you review the email logs).
