Form Thank You Page Not Working (I'm receiving email)

I am at a loss. I have tried correcting the path that my "Thank You" page should be pointing to, but no matter what I try, I continue to get a 404 error page when I hit "submit" on my form. I am getting the email immediately, so thankfully that part works, but need help on the code for the path. Anyone, anyone?
